alright i'm not sure of all the A1 and stuff so i'll explain the way i know how...

someone on red team gets a slash. Goes into box. Ball starts right under center X. Middie starts with ball and teammate facing him 2 or so feet apart. Ref today said that we must be 5 yards apart when we start the clock again? is this true even after letting the goalie know who is starting with it?

Would work the same with a timeout kind of deal...

yes, after a penalty or an out-of-bounds ball, the oponent cannot be within 5 yards of the carrier until the whistle is blown.

Why would your own teammates be so close?

It's real simple - NO other player can be within 5 yards of the person with the ball - teammates included. It's in the rulebooks.
