From Goalie, i can see alot of weird stuff go on.
When the ball is awarded next to the subs box, here is my idea
(if any penalties apply let me know)
1. a middie A1 is (legally) 5 yards away from player B1
2. B1 is on his defencive side, next to the subs box
3. the ref gives ball to B1 after out of bounds type thing
4. A1 lines up against box
5. when play starts, A1 jumps in box
6. A2 is waiting in box and the second A1 comes in he jumps out right next to B1 and knocks him over before he knows whats going on
its like teleporting
only it doenst defy physics!!!

First of all, quantum teleportation does not defy the laws of physics. It's a large-scale teleportation that's an issue due to the uncertainty principle. If you want a good explanation of this sort of thing in layman's terms, read (or listen to the audio book) The Fabric of the Cosmos. The unabridged CD book is 22 CDs, but I learned a lot of physics while driving to and from lacrosse games.
But this technique will not work if the officials do their job correctly, since when you're near the table area the player with the ball needs to start 5 yards from the sideline. Your scenario is probably one of the reasons for this.

IDK if this is a rule but I've always heard that when you are released from the box on a penalty, you cannot go directly out of the box and hit somebody. You have to play the ball, even during a loose ball situation.

You can go right out and hit someone. What you can't do is go out and hit an unsuspecting player.
And besides:
ART. 1 . . . Free play - Whenever a player has been awarded the ball for any reason. Under such conditions, no player may take a position closer to him than five yards.
That doesn't just say players on the field, so the guy in the box waiting to sub still couldn't be within 5 yards.

i doubt a ref would call someone whos out of bounds

prob so but i mean i know we've had guys (diff scenario) come off on the defensie transition and our guy is on the other side of the box jumps out as he gets in and now is ahead of the other team's guy with teh ball, smacking him silly or stopping him.
