Go in front of a mirror with your stick (no ball) and go through your throwing motion with your dominant hand several times. Then, switch to your off-hand and go through the throwing motion several times, and make sure that it looks identical to your dominant hand.

Make sure you have your hands in identical postion and have the same throwing motion with both hands. Try putting tape down on your shaft so you know where your hands should be when throwing lefty.

You also need to reverse your foot work. A righty would step into a standing shot or pass with his left foot. A lefty would step in with his right foot. For a running shot or pass you tend to lead with the foot (side) you shoot from.

most likely, your hands are too close into your body, or your hands are getting ahead of your shoulder and hip rotation
