I think this is right or nearly so: there are a few different ways; depends on where the ball was and what type of foul or if it went OB. ball never restarts inside the box -- always move it out (I was told also never to start it at the center top; always top left or right or, for fouls where defense gets possession when ball was in the box, goal line extended outside the box). you need to check the rules as to exactly where the ball is restarted; depends on where the ball was and what type of foul. the man awarded the ball (if after out of bounds, he steps in bounds; the ball always restarts inbounds) waits for the restart whistle. the defender(s) must be set at least 5 yards away from him. if they are too close tell them to back 5 yards off. if they dawdle etc (or ignore you hoping you'll start anyway) you can call them for delay of game (30- sec technical). they start on the whistle. if you are starting the ball in front of a bench, bring the man with ball in 5 yards from the bench so you get him away from it. as you blow whistle you also do a windmill arm motion to signal restart of time. next time you observe a game watch the refs and you'll pick up a lot to help with what you read in the rules.

You always start on the field of play, so, no, it's not like a throw-in for soccer or basketball. If it goes out of bounds, you step 1 yard in from the sideline--or 5 yards if you're near the table area--and everyone must be at least 5 yards from you. When the whistle blows, you can run with the ball or pass it (or shoot, I guess, but you'll likely be too far away for that to make any sense).
