NCAA rules...but I think NFHS are the same, too
Loose ball in the crease when official blows whistle. Under what conditions would ball be awarded AP and under what conditions would the ball be awarded to the defense laterally from the goal outside the attack goal area?

Any penalty to the offense would be awarded to the defense laterally, ball caught in the net would be awarded to the defense.
Matching penalties would be awarded A.P., ball mired in the mud A.P., goal keeper loses equip-A.P.

Inadvertent whistle while ball is loose inside the crease, ball awarded to defense outside the attack area, at least in NCAA. There are no inadvertent whistles in NFHS lacrosse games . . . . :chuckle: (Rule 7-13)
Ball mired in the mud inside the crease or ensnared in the goal netting, ball awarded to defense outside the attack area. (Rule 4-20)
Ball stuck in GK's equipment within the crease, ball awarded to defense outside the attack area. (Rule 4-7)
GK breaks or loses equipment while ball is loose in the crease. . . . I can't find a definitive statement. However, looking at the above, it sure seems like the answer would be "ball awarded to defense outside the attack area
Oh, and tjslax is right that on any single penalty or non-time-serving violation, the ball will restart outside the attack area (maybe even across midfield if there's a personal foul by the offense). On simultaneous fouls, see rule 7-6.
