Just saw this occur and I am not certain it was handled at all correctly...both coaches seemed rather stunned.

Team A on a stall warning. Ball has not left the box. Suddenly, a whistle blows. Its inadvertant. Play stops as a result. Ball is awarded to A OUTSIDE the restraining box. Warning off. Play resumes.

Am I correct in assuming that play should have been restarted INSIDE the box, warning remaining in place?

No. Play can never restart inside the box unless there is a timeout on the endline or an out of bounds (shot, pass, step) on the endline. If there was a stall warning in effect with no intervening possession by team B, then team A will have 10 seconds to get the ball in the box and then the stall warning will be in effect again.

I had this similar situation yesterday. The ball was in the box and the other official thought he had offside against the team in possession so he blew his whistle. He was wrong about the offiside so we restarted with the same team in possession outside the box.

And as LaxRef points out, after they get it back in within 10 seconds the stall warning is put back on, since there was no intervening possession by the other team.
