...NFHS Rule 1-6 "The circumference of the crosse handle shall be no more than 3-1/2 inches." May be invoked.

With this rule in mind, what does the NCAA rulebook say about this? I checked the downloadable .pdf rulebook but found nothing.

The NCAA rule is identical, although with regards to endcaps I have heard that endcaps are not to be considered when measuring the circumference of the handle. This, however, does not address the doughnuts that some players are placing at the end of their crosses.

NCAA 06, p18:
The circumference of the crosse handle shall be no more than 31/2 inches
and the handle must be straight.

I did catch that rule, but it didn't seem to specifically address the endcap, just shaft circumference.

what defines an endcap? Is it just a plastic cap, or do homemade tape ends count as well?

And if they don't address it, there are no restrictions. Unless, of course, you do something so crazy as to be ruled USC.

Since they specifically say you can use an adequate amount of tape to cover the opening and prevent injury, I would consider the tape to be an end cap.
