NFHS multiple players in P box

I am a new official and am having a hard time finding the answer to this question.
Team A has 3 in P box and A4 gets a penalty. What is the correct procedure?
How many players on the field for Team A and where for the restart and is A4 in the P box? How does in differ from NCAA?

NFHS: There's nothing in the book about it because you just put everyone that gets a penalty in the box. You could theoretically have 10 guys from one team in the box, although you'd probably call the game before it got to that point. (If Team B has 11 guys in the box, they should be playing down 11 men, so does Team A get to put an extra guy on the field? :chuckle:)

In NCAA, it's complicated:

A.R. 1. A1, A2, A3 and A4 receive one-minute releasable penalties which are all reported
during the same dead ball. A1, A2 and A3 begin to serve their penalties at the same time,
while A4 waits in the bench area immediately next to the scorer's table but not in the
special-substitution area. Team A restarts play with seven players on the field. (1) Team
B scores a goal after 30 seconds. (2) Team B does not score a goal in the next minute.
RULING: (1) A1, A2 and A3 are released, and A4 begins serving his one-minute
penalty. (2) A1 and A2 may enter the field. A3 must exit to the bench area, and A4 steps
into the special-substitution area and begins serving his penalty. Team A is now playing
with nine players on the field. A3 may now substitute into the game in the normal manner,
if desired.

It is a question on the USLax HS test. The answer choices are:
a) Team A will have 4 in P area, 3 on O side& 3 on D side
b) Team A will have 4 in P area, 3 on O side & 4 on D side
c)Team will have 3 in P area, 3 on O side & 4 on D side. A4 will be confined to the bench area until the 1st penalty expires & then A4 will begin serving.
d) A1 will not serve the unless it is an USC.

I picked c and it was wrong for HS, but I it right for NCAA
